3/8/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, When QMHP-C came to visit, Client was seen at the door in excitement. Client enjoys spending time with her counselor. Client began a conversation discussing parts of her day, good and bad. Client admitted that some of her decisions can put her in tough situations and everything is not the way she perceives it at which mother reports behaviors can last up to 5-10 minutes in duration when redirected by her to complete a task. Behavior: Client appeared to be withdrawn at times within the session when discussing with her mother her behaviors as well as her on ways she can better communicate her thoughts. The withdrawal lasted up to 2 minutes in duration at the time of the session. Intervention: QMHP-C refocused Client as she was observed becoming fidgety and impatient. QMHP-C reviewed and gave feedback regarding ADHD and its causes and affect toward Client well-being and academic functioning. QMHP-C provided Client with support and encouragement. Intervention: QMHP-C used positive regard as she told Client that she appeared to have positive home management early in the day. QMHP-C informed Client of more acceptable and expected responses to situations that would normally lead Client to be triggered and display inappropriate, negative behaviors while in the home. Intervention: QMHP-C assisted Client to behave in more positive ways by addressing the conflict she was facing with her mom. QMHP-C informed Client of replacement behaviors in efforts to decrease attention seeking behaviors Client often is involved in. QMHP-C modeled appropriate ways for Client to get positive attention from her family. QMHP-C helped Client identify what behavior she needs to decrease and avoid daily. QMHP-C discussed progress and expectations that are working and not working in the home so that Client and parental guardian have a high expectation and good example of where Client is mentally now compared to where she used to be. Response: Client was able to be redirected successfully. Client was able to control her impulsive actions. Client was observed having positive interactions with everyone in the home. Client communicated that she feels that she will be better behaved, due to consistent support in the home with mom and seeing QMHP-C 4x a week. Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Client learned to avoid negative and defiant behavior and comply to rules. Client was seen following directions. Response: Client expressed appreciation to QMHP-C by smiling and giving good eye contact. Client verbalized that she’s able to focus better when it is quieter and more peaceful. Client took her mind from thinking and doing to a place of concentration, which allowed her to focus on one thing at a time instead of being all over the place and getting easily distracted. Client demonstrated more acceptable responses, avoiding anger provoking situations in the home. Client utilized the resources provided and appeared to be doing better with managing symptoms of her disorder. Client was able to maintain her composure when she was confronted with the possibility for an outburst. Client was able to complete her work without needing to redirect her to focus and pay attention in the session.
